JOB DESCRIPTION

SUMMARY: Assesses for service needs and aids older adults referred from the community and from within Senior Resources of Guilford.

REPORTS TO: Chief Operating Officer

ESSENTIAL DUTIES AND RESPONSIBILITIES (Other duties may be assigned.)

  1. Supervises two staff social workers.
  2. Interviews, in their homes, individuals referred to the Senior Resources of Guilford Meals On Wheels Program. Gathers information regarding client’s physical and mental impairments, financial, and social supports. Determines client’s eligibility for the Meals on Wheels Program.
  3. Reassess the eligibility of individuals for the Meals on Wheels Program as scheduled.
  4. Performs Case Assistance if the need presents itself during the assessment/reassessments.
  5. Accepts referrals from Senior Resources of Guilford SeniorLine or other program staff for assessment and review.
  6. Identifies medical, psychological, or social problems contributing to client's situation, and evaluates these and client's capacities.
  7. Develops individual service plans outlining client needs and activities intended to resolve the client’s situation.
  8. Refers clients to community resources and other organizations.
  9. Counsels clients individually or with their family regarding service plans for meeting needs, and assist client in contacting community resources to improve their situation.
  10. Compiles records and prepares reports.
  11. Accesses and records client and community resource information.

Qualifications: BSW degree with experience working in community based human services agencies.

Job Type: Full-time

Pay: $34,618.00 - $38,000.00 per year

Job Description

Senior Resources of Guilford is seeking a Registered Nurse to provide mental health training, consultation, and marketing for priority agencies for the Geriatric Adult Mental Health Specialty Team (GAST). The candidate should have mental health and demonstrated training experience, good oral/written communication, organizational and time management skills. Experience working with seniors is preferred.

DUTIES AND RESPONSIBILITIES

  • Work in coordination with other GAST team members to select, create and develop appropriate training materials and curriculum that will enhance the skills and techniques of staff and informal caregivers of older adults with mental illness.
  • Provide educational on-site trainings for staff on topics such as recognizing symptoms of mental illness, assessment of behaviors, behavioral interventions, effective communication, and medication issues.
  • Assist with the creation of marketing materials for GAST through collaboration with other GAST team members. Assist with marketing GAST by providing community presentations, making telephone calls, and face-to-face contact in addition to contributing to the GAST newsletter, Facebook, and web page presence as assigned by Team Leader.
  • Establish and maintain positive working relationships with the Family Justice Committee, Area Agency on Aging Ombudsman, and other community contacts.
  • Fulfill continuing education requirements as necessary to maintain licensure and credentials.
  • Attend meetings both within the agency and throughout the community as requested.
  • Other duties as assigned.

Certifications Required

Registered Nurse with current licensure in North Carolina

KNOWLEDGE, SKILLS AND ABILITIES

  • Strong organizational, time management, and communication skills.
  • Ability to work cooperatively and effectively with staff, service providers, and senior adults.
  • Have reliable transportation and a valid NC driver's license.
  • Basic computer skills for using e-mail and Office applications.

EDUCATION AND EXPERIENCE

  • Current RN License in North Carolina required.
  • Experience working with older adults preferred.
  • Experience in the nonprofit field preferred.

Integrative Services Program Director

Integrated Services Program Director

REPORTS TO: CEO

HOURS: Monday-Friday; 8:30 to 5:00

DUTIES AND RESPONSIBILITIES:

Provide coverage for the day-to-day operations of the SeniorLine and Senior Wheels services by entering contact data, follow-up data, and other information into program related databases in a timely manner.

Assist with completion of monthly, quarterly, and annual reports.

Respond to all requests for information in a timely manner.

Assist with fundraising efforts.

Other duties as assigned.

Senior Wheels:

Coordinates ride requests between clients and drivers daily.

Input ride requests and client information into appropriate database.

Send information packets to potential Senior Wheels riders, and ensure a signed, dated “Client Release Form” is obtained before scheduling the client’s first ride.

Recruit drivers by engaging community and faith-based organizations.

Represent Senior Resources of Guilford to groups as speaker for health fairs, recruitment or appreciation events.

SeniorLine

Receive telephone calls from older adults and those calling on their behalf and provide appropriate information and referrals to meet the client’s needs.

Provide problem solving options and answer questions regarding agency programs and community resources. Invite a call back if a referral does not solve the program.

Follow-up with individuals referred to community agencies to ensure the individuals’ needs were met and that the information given to the caller was correct and appropriate.

Enter contact data, follow up data and other information in the iCarol database in a timely manner.

Maintain current knowledge of issues and resources related to older adults.

Maintain contact with representatives of other organizations to exchange and update information on resources and services available.

Assist with scheduling appointments for the Volunteer Income Tax Assistance (VITA) Program.

Assist the Integrative Services Program Director with the implementation of the Heat Relief Program by checking client eligibility and maintaining client records when fan requests are received.

Obtain and maintain appropriate certifications, including but not limited to, the Alliance of Information and Referral Systems- Information and Referral Specialist for Aging/Disability.

KNOWLEDGE, SKILLS, AND ABILITIES:

Excellent written and verbal communication skills.

High proficiency in use of computer databases.

Ability to understand and work well with older adults and caregivers.

EXPERIENCE/EDUCATIONAL QUALIFICATIONS:

Bachelor’s degree in Social Work, Human Services, or related field; or equivalent experience.

Experience working with older adults preferred.

Experience with non-profit or human services agencies preferred.

CERTIFICATION REQUIREMENTS:

Certification through the Alliance of Information and Referral Systems will be required as soon as eligible.

Senior Resources of Guilford is seeking a part-time driver to assist with Meals on Wheels delivery in High Point. You will deliver approximately 50 meals a day to homebound seniors. The timeframe for each shift is 8:30 am - 2:20 pm (approximately). At 8:30, you will pick up the truck loaded with meals on Landview Dr/Wendover Ave in Greensboro, deliver the meals and then return the truck. The ideal candidate would be willing to work 2 days a week, plus as needed due to other drivers' sick/vacation time. Sometimes the driver is asked to work a longer shift to take the truck to the service center for maintenance. The applicant must be able to get in and out of a truck (Chevy Colorado) approximately 50 times per shift for meal delivery. Some deliveries require stairs, inclined driveways, etc. Applicant must have a valid North Carolina driver's license, clean driving record, no criminal history and willing to submit to a background check. No CDL required.
