Lead Social Worker

JOB DESCRIPTION

SUMMARY: Assesses for service needs and aids older adults referred from the community and from within Senior Resources of Guilford.

REPORTS TO: Chief Operating Officer

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  1. Supervises two staff social workers.
  2. Interviews, in their homes, individuals referred to the Senior Resources of Guilford Meals On Wheels Program. Gathers information regarding client’s physical and mental impairments, financial, and social supports. Determines client’s eligibility for the Meals on Wheels Program.
  3. Reassess the eligibility of individuals for the Meals on Wheels Program as scheduled.
  4. Performs Case Assistance if the need presents itself during the assessment/reassessments.
  5. Accepts referrals from Senior Resources of Guilford SeniorLine or other program staff for assessment and review.
  6. Identifies medical, psychological, or social problems contributing to client's situation, and evaluates these and client's capacities.
  7. Develops individual service plans outlining client needs and activities intended to resolve the client’s situation.
  8. Refers clients to community resources and other organizations.
  9. Counsels clients individually or with their family regarding service plans for meeting needs, and assist client in contacting community resources to improve their situation.
  10. Compiles records and prepares reports.
  11. Accesses and records client and community resource information.

Qualifications: BSW degree with experience working in community based human services agencies. 

Job Type: Full-time

Pay: $34,618.00 - $38,000.00 per year

Registered Nurse - Geriatric Adult Mental Health Specialty Team

Senior Resources of Guilford is seeking a Registered Nurse to provide mental health training, consultation, and marketing for priority agencies for the Geriatric Adult Mental Health Specialty Team (GAST). The candidate should have mental health and demonstrated training experience, good oral/written communication, organizational and time management skills. Experience working with seniors is preferred.

DUTIES AND RESPONSIBILITIES:

1. Work in coordination with other GAST team members to select, create and develop appropriate training materials and curriculum that will enhance the skills and techniques of staff and informal caregivers of older adults with mental illness.

2. Provide educational on-site trainings for staff on topics such as recognizing symptoms of mental illness, assessment of behaviors, behavioral interventions, effective communication, and medication issues.

3. Assist with the creation of marketing materials for GAST through collaboration with other GAST team members. Assist with marketing GAST by providing community presentations, making telephone calls, and face-to-face contact in addition to contributing to the GAST newsletter, Facebook, and web page presence as assigned by Team Leader.

4. Establish and maintain positive working relationships with the Family Justice Committee, Area Agency on Aging Ombudsman, and other community contacts.

5. Fulfill continuing education requirements as necessary to maintain licensure and credentials.

8. Attend meetings both within the agency and throughout the community as requested.

9. Other duties as assigned.

Certifications Required:  

Registerd Nurse with current licensure in North Carolina

  • Ability to work cooperatively and effectively with staff, service providers, and senior adults.

  • Have reliable transportation and a valid NC driver's license.

Rural Outreach Coordinator

JOB TITLE: Rural Outreach Coordinator

RESPONSIBLE TO: Senior Center Director (Julie Silverman)

JOB OBJECTIVE: Provide coordination and programming to 7 rural outreach sites targeting older adults in Guilford County. Sites are located in  Greensboro, Oak Ridge, Colfax, Summerfield, and Stokesdale

SCHEDULE: Part-time schedule requiring an average of 20 hours per week.

DUTIES AND RESPONSIBILITIES:

  • Plan, implement, and attend monthly rural outreach activities with speakers or entertainment.

  • Attend and set-up 2 men’s coffee and conversation events monthly in Summerfield.

  • Source new speakers/activities that appeal to older adults.

  • Supervise the Rural Outreach Site Manager.

  • Create marketing materials for monthly events and provide outreach potential participants.

  • Develop new rural outreach sites to expand the program.

  • Maintain accurate data regarding participation, budget, and mileage.

KNOWLEDGE, SKILLS AND ABILITIES

Community Nutrition Site Manager (Part-time)

Job Objective: Good management of Congregate Nutrition Site that meets the needs of the older adults in accordance with Title III guidelines. Responsible for the supervision of all functions related to food, program activities, and transportation when applicable.

Site Manager Hours: Part time, currently 2-3 days/week (Tuesday 8:00 am - 12:30 pm & Thursday 8:30 am - 10:00 am, plus covering Site Managers as needed)

  • Monthly site manager meeting with Nutrition Program Director on the last Friday of the month, from 2-3pm

  • Additional trainings to be announced as needed

Site Manager Essential Duties and Responsibilities:

Ensure that Federal and State guidelines are followed.

Submit collections of program income to the Finance Department weekly. Ensure that the form is correctly completed and that a funds are counted daily by a volunteer and site manager in each other’s presence.

Submit required weekly and monthly reports accurately and in a timely manner.

Oversee the proper use and care of fire extinguishers, follow health regulations regarding fire drills and safety inspections, and sanitation inspection reports. Provide the Nutrition Services Director with all inspection reports upon completion.

Strive to recruit program participants and increase enrollment at site.

Attend monthly site Manager Meetings and other meeting as required scheduled by the Nutrition
Services Director.

Recruit, train, an supervise volunteers and Senior Aides who can assist with meal service and act as a substitute in the site manager’s absence.

Ensure that all required documents are posted at the site. Menus, calendar of events, sign in sheets,
Evacuation plan, emergency phone numbers, and cost sharing information.

Ensure that volunteers follow correct procedures for food handling, food temperatures and serving utensils. (Handwashing, plastic gloves, food portions, food temperatures).

Responsible for checking food on arrival to be sue it matches the menu and responsible for checking food temperatures in compliance with Federal regulations.

Maintain a positive relationship with Food Vendor personnel for purposes of trouble shooting.

Responsible for notifying the Nutrition Services Director when there are changes in transportation needs and/or transportation difficulties.

Ability Ensure that the Nutrition Services Director is notified of all problems and concerns at the meal site including emergencies, meal discrepancies, and client incidences.

Responsible for assessing program needs and interests, and to plan, implement, and evaluate program activities.

Responsible for checking mailbox at and distributing flyers to participants in a timely manner.

Knowledge, Skills and Abilities

Ability to work cooperatively and effectively with staff, service providers, and senior adults

Ability to carry and lift pans of food

Ability to organize and supervise volunteers

Ability to drive agency van for offsite trips if needed

Ability to adapt oneself and participants to changing conditions, to solve problems with a positive attitude.

Knowledge of community resources and of food service and sanitation procedures.

Full Job Description

The Evergreens Lifestyle Center, a program of Senior Resources of Guilford, is seeking a Nepali translator for 2 hours monthly (the second Friday of each month from 12-2pm) for our Refugee Outreach Program. Occasional additional opportunities will be offered as available. This individual will provide translation for refugee seniors, staff, and guest speakers during outreach events that take place at the Evergreens Lifestyle Center. Activities may include translating for educational programming, games, or case management.

Qualifications:

  • Demonstrated fluency in Nepali and English
  • Previous interpretation experience preferred
